At the conclusion of all Twelve Step model treatment programs, the advice given as you exit the facility is “ don’t drink, and go to meetings” and things will get better.

The reality is that a Twelve Step program works if you work it. Recovering alcoholics and addicts, who have long-term sobriety maintain their sobriety by not drinking/using and going to meetings.
Aftercare is a clinical term used by discharge planners at primary care treatment facilities. Very simply it means “ What do you do now?” You are probably being advised by the discharge planner to go to a sober living residence immediately after leaving primary care. The reason for this suggestion is to
make sure that you maintain your enthusiasm about your new sobriety.
Returning to your old “stomping grounds” immediately after primary care is a very big risk to take.
